Take the important step of completing an Advance Directive, which includes both a Power of Attorney for Healthcare and a Living Will. These documents will help ensure your wishes are respected if you’re unable to make medical decisions for yourself.

By signing this Consent for Medical Care Agreement, you authorize Elumimed to provide your medical treatment, share your health information as needed, and receive payment for services rendered. Your signature is required before treatment, except in emergency situations.

Illinois Warm Line
If you or one of your family members has mental health and/or substance use challenges and would like to receive support by phone, call the Illinois Warm Line. This is not a crisis hotline but is a source of support as you recover or help a family member to recover. Hours of operation are Monday through Saturday, 8 a.m. to 8 p.m., except holidays.
